COURSE OVERVIEW

Effective incident investigation is a key activity of SMS and essential to companies that want to understand their internal risks and safety performance. The effective investigation and analysis of incidents can lead to direct improvements in a company’s operations. The Incident Investigator course teaches students the essential elements of safety incident investigation in the context of an aviation SMS. It first teaches students to identify events that may contain significant risks. The focus then shifts to preparing for and conducting an investigation. It concludes by teaching how to analyse the findings of an investigation and how to communicate the outcomes. The exercises support the course content and are designed to provide practical training in key elements – for example identifying causal factors. There are also completed examples as reference.

The Incident Investigator course has been specifically designed for people in small to medium-sized organisations who need the skills to conduct their own safety investigations, or to oversee safety investigations. This can include management at all levels and anyone involved in, or supporting, an investigation. The course content aims to provide this essential skill without over complicating the topic.
